Notice of Coronavirus Quarantine to Employer

This document is a simple letter that an employee can use to notify their employer that they are under quarantine due to the coronavirus, or COVID-19.

During the global pandemic, many employees are facing situations where they have either been exposed to someone with the virus or they may have even tested positive for the virus. Most Americans are self-isolating, but for some, a complete quarantine is needed. This may have been suggested by a medical professional or it may simply be a measure that the employee has decided to take.

This letter notifies the employer that the employer requires time off from physically going into work.

 

How to use this document

This document should be filled out by the employee. It does not contain a lot of details, as the CDC has warned employers that requiring a doctor's note may not be prudent at this time that the nation is dealing with COVID-19. Requiring a doctor's note may create a situation where people are overwhelming medical providers, or going into work sick if they can't get a note.

The document only asks the most basic details, including name and contact information, as well as currently foreseen dates of quarantine. The text of the document does note that the quarantine dates may be variable.

When this document is complete, it should be signed, either electronically or with ink, and sent to the employer. The employee should keep a copy.

 

Applicable law

There is no specific law governing what needs to go into a notice to an employer about quarantine. The employee should give the employer as much information as they feel comfortable with.
